If you are travelling abroad, please make sure you complete a travel assessment form at least 10 weeks in advance to arrange any vaccinations that may be necessary.

To help the Travel Nurses assess your travel needs, it is important that they receive your completed assessment form before your appointment.
We may need to order the vaccinations required for your destination. Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
